STAFF ONLY - COVID-19 form
Townsville Hospital and Health Service staff who become COVID-19 positive or are a close contact of a COVID-19 positive person can use the below form to alert their service group directors.
Do not proceed further unless you meet the close contact or positive Covid-19 case definitions.
Positive case:
- An individual who has received a positive COVID-19 test result
Close contact:
- You live with someone who has received a positive COVID-19 test result (ie: partner, housemate or visiting family member)
- You’ve been with someone who has COVID-19 for more than 4 hours in their accommodation
Please note: you DO NOT meet the definition of a close contact if you have had contact with a positive case in the workplace.
