Accessing your personal medical record

Regardless of how your record is held, either paper or electronic, you can apply for access.

You can do this by speaking to your healthcare team directly or by completing the application form and sending it to the release of information unit.

Medical records will come directly from the individual health facility. For more information or, to follow up on a request, please contact each facility directly.
