Updating your records or contact details
You may need to update your address or contact details, or you may disagree with a fact or opinion documented in your record. If you wish to apply for amendment to incorrect information within your record or you believe information is included in your record which is not about you or that is not relevant to you, you should contact the Release of Information Unit. If you want to amend your details you can do so in person, by telephone or in writing.

Changing your details by phoning us
It is important that we can contact you, particularly if you are waiting for an appointment or service to become available. If you want to update your contact details or change your general practitioner (GP) information, you can phone us and, after answering a few security questions, staff will update your details.
Changing your details by writing to us
If you request to change your details in writing (letter or email) you will need to provide information so that we can identify you. This can include your full name, date of birth and Medicare number as well as the old and current versions of any information you wish to change, for example your old and current address.
If you do not provide us with sufficient information, Townsville Hospital and Health Service staff will attempt to contact you before amending your details.
